How to complete your application:

  1. Print out the BLANK application form (choose the format which is easy for you to print). If you cannot print the form, copy it VERY CAREFULLY from the screen on a blank sheet of paper (white printer paper or white photocopy paper is best).
  2. VERY CAREFULLY review this EXAMPLE to see how you should complete your own application. (You may print it out if you wish.) All details are VERY IMPORTANT.
  3. Write YOUR information on the BLANK application form and sign it. ENCLOSE (DO NOT attach) your photograph.
  4. Send your application form + your photograph + payment to: Six Interactions / G.C.P., Suite 330, 5525 West Blvd, Vancouver, B.C.

  5. If you are married, your spouse should complete another application, following the same steps (1. through 4. above). If both you and your spouse send your applications, your chance to become permanent residents of the U.S.A. will be twice as high.
