What should you do?
1. Print out one application form from our site for yourself and if
you are married, print out another one for your spouse (wife or husband).

Using your browser's Print function, print the form on white printer paper. The entire
form must fit on one side of the page. If you cannot print the Blank Form, write
the entire text very carefully by copying it from the screen on a blank sheet of
white paper (white printer paper or white photocopy paper is best).
2. To see how you should complete the Blank Form, click here. (You may print or copy this Example
page if you wish.) Review this Example very carefully, as all details are very
important.
3. Very carefully, complete the Blank Form. (If you are married, your
spouse can complete a second Blank Form. This will double your chance of winning.) Only
use English characters. Please print or typewrite. If you cannot print or typewrite,
handwrite very carefully in block letters. Make sure everything you write is easily
legible. Do not forget to sign the form. (Sign in your own native alphabet.) Only you
must sign your own application, not your spouse or children. If your spouse is
completing a second form, she/he must sign that second form herself/himself. The entire
form (including the signature) must fit on one side of the page.
4. Check the form for completeness and accuracy.
5. Get one recent photograph (color or black-and-white) for yourself,
one for your spouse and one for each child. You must include a photograph for each
unmarried child under the age of 21 years, including all natural children as well as all
legally-adopted children and all stepchildren, even if you are no longer legally married
to the child's parent, and even if the child does not currently reside with you and/or
will not immigrate with you. All photographs must be 50 mm x 50 mm (2 inch x 2 inch) in
size. On the back of each photograph, write the name and date of birth of the person
shown. Attach each photograph with tape, do not staple to the application form.
6. Air mail the form(s), photograph(s), and payment to:

The decision regarding your case will be made by the U.S. Government.
Please understand your processing fee is not refundable once received, regardless of
the outcome of your case.
Upon the completion and submission of your application, a
confirmation letter can be sent to you by air mail. Please enclose a self-addressed
envelope if you wish to receive such confirmation letter.
The U.S. Government will inform you between April and July 2003, in
case you were selected for permanent residence in the U.S.A. Only successful applicants
will be informed.
After submitting your application, our office will not be able
to provide any information about the status of your application. Please do not write to us
with questions about your application after you receive your confirmation of submission.
Inquiries regarding submitted applications will be ignored and discarded.
Please remember that we must receive your application form(s),
photograph(s), and payment no later than October 22, 2001, to guarantee a timely
submission of your application(s). Although we may process requests after that date, we
cannot guarantee that the U.S. Government will.
Applications received after the closing date of this program can be
submitted in the next run of the Diversity Visa program at no additional cost, but only
with the applicant's explicit written consent. No refunds will be given for such late
applications.
